Terms and Conditions of Service

Effective Date: September 1, 2025

Introduction

College Concierge Medical, LLC (“Medical,” “we,” “our,” or “us”) provides concierge-style medical care services to college students, including but not limited to telemedicine, house calls, and in-office visits (“Services”). These Terms and Conditions (“Terms”) govern the relationship between the Medical and the subscribing student (“Patient”) and/or their parent or legal guardian (“Subscriber”).

By enrolling in or using our Services, you acknowledge that you have read, understood, and agreed to these Terms.

Nature of Services

  • Our Services are designed to provide primary and urgent non-emergency medical care.
  • We are not a replacement for emergency medical services. Patients experiencing life-threatening emergencies should call 911 or go to the nearest emergency department.
  • Services may include:
    • Telemedicine consultations
    • Dormitory, apartment, or in-home visits
    • In-office appointments
    • Preventive, urgent, and follow-up care

Eligibility

  • Patients must be enrolled college students at an eligible institution.
  • Patients under 18 years of age must have parental or guardian consent before receiving Services.

Membership and Fees

  • Services are provided on a concierge membership model.
  • Membership fees are billed on a monthly or annual basis.
  • Fees cover access to the Services but do not include costs for prescriptions, laboratory tests, imaging, or referrals.
  • Additional services, beyond primary care, are available for a separate fee.
  • All fees are non-refundable except where required by law.

Insurance

  • The Medical does not bill or accept health insurance.
  • Patients and Subscribers are responsible for all charges related to outside services (specialists, lab work, imaging, pharmacy).

Telemedicine Services

  • By using our telemedicine platform, you consent to the use of secure electronic communications for diagnosis, treatment, and follow-up.
  • Limitations of telemedicine include the inability to conduct some physical exams and the potential for technology-related interruptions.

Scheduling and Cancellations

  • Appointments may be scheduled via phone, email, or our secure patient portal.
  • We request at least 24 hours’ notice for cancellations.
  • Missed or late-cancelled appointments may result in a fee.

Privacy and Confidentiality

  • We comply with all applicable HIPAA regulations regarding patient privacy and confidentiality.
  • Please review our Notice of Privacy Practices for more information.

Limitations of Liability

  • The company and its providers will make reasonable efforts to provide quality care.
  • However, we make no guarantees of specific outcomes.
  • To the fullest extent permitted by law, the company is not liable for indirect, incidental, or consequential damages related to the use of Services.

Patient Responsibilities

Patients agree to:

  • Provide accurate and complete medical history and information.
  • Provide identification at each visit, upon request.
  • Use Services only for lawful and appropriate purposes.
  • Follow medical advice and treatment plans as recommended.

Termination of Services

  • The company reserves the right to terminate membership or deny Services in cases of:
    • Non-payment
    • Misuse of Services
    • Threatening or inappropriate behavior
  • Patients may terminate their membership at any time; however, fees are non-refundable.

Governing Law

These Terms shall be governed by and construed under the laws of the State of Arizona.
